Pre-Season Information for Parents & Players
How to Register:
All participants in athletic programs at Armstrong High School must be fully registered prior to the athlete's first practice and/or tryout. Registration forms can be picked up at the Armstrong Activities Office after August 1st or downloaded from our website. Registration forms and participation fees can be returned to the Activities Office beginning the first week of August. Office hours are 7:00 am to 3:00 pm.
Click here to download the ATHLETIC REGISTRATION FORM or ACTIVITY REGISTRATION FORM. These forms must be completed once per school year.
An Emergency Card must be completed each season of participation. Click here to download the ACTIVITY EMERGENCY CARD.
Athletes must provide proof of a sports qualifying physical exam, done within three calendar years, prior to the first day of practice. Check with the athletic secretary or the school nurse to confirm that a current physical is on file before the sports practice season starts. If the student does not have a copy of a sports qualifying physical on file with the school nurse, the parent/guardian should call their doctor/clinic and get a copy. If the doctor's office needs a form to fill out, a copy of our physical exam form may be picked up in the athletic office or downloaded from the Armstrong website. If the student has not had a physical in the last three years, then the student must get a physical. It is a Minnesota State High School League rule that all students participating in sports must have a current up-to-date physical on file with the school at the time of registration. (The physical must state whether the student is able to participate in sports.) Click here to download the HEALTH AND PHYSICAL EXAMINATION FORM: 2018-2019 PhysicalExam.pdf
